Introduction
Potential Failure Mode & Effects Analysis (FMEA) is an established risk assessment methodology for systematically identifying the potential failure of products or processes, assesses the impact of the failure, and develop action plans for mitigation. FMEA is applicable to products design and manufacturing processes development.  A well-developed and comprehensive FMEA is proven to greatly reduce the cost of poor quality and which will continually improve the quality of processes or products.

The course is aligned with the latest version of AIAG’s FMEA Reference Manual (4th Edition) and the core tools requirements in IATF 16949.

  • Introduction to FMEA
    • To provide a fundamental understanding of what FMEA is, types of FMEA, where, why, when it is used, FMEA sequence, general principle, Risk Priority Number (RPN) Calculation.
  • FMEA - Getting Started
    • Defining key date, identifying FMEA members, determine scope of FMEA
  • Development of Design FMEA (DFMEA)
    • Determine design function, potential failure mode and failure effects - how to determine Severity (S) - classification of special product characteristics -  identification of potential cause and how to determine the Occurrence (O) - current design control on prevention and detection types - how to determine Detection (D) -  RPN computation - improvement action -   re-computation of RPN – Exercise.
    • Includes: Block diagram development, SOD ranking table interpretation, how to reduce SOD and application workshop.
  • Development of a Process FMEA (PFMEA)
    • Defining process function, potential failure mode and failure effects - how to determine Severity of effect (S) - classification of special characteristics - identification of potential cause and how to determine the Occurrence (O) -  current process control - how to determine likelihood of Detection (D) - RPN computation -  improvement action - re-computation of RPN - Exercise.
    • Includes Process Flow development, RAG analysis, SOD ranking table interpretation, how to reduce SOD and application workshop.

This course is designed for front line managers, engineers or executives who are involved in the developing and implementing FMEA.
